This is a tough one but it is not impossible. My suggestion is similar to Davits; search the support group for conversations that might be helpful to you - also use the program. The sessions on worry stopping and relaxation techniques might be particularly helpful for you now. Also, look at this as a challenege. If you fail it will feel pretty bad at the time but you will learn from it and you will move on. We will help you work through it. Check out the ten questions to challenge negative thinking too - this can really help to put everything into perspective.
You also would be surprised how understanding professors can be when it comes to anxiety. Anxiety is a very common issue for many so I am sure this won't be the first time they have interacted with an anxious student. If you want you can even mention this to your teacher - that you are currently working on your anxiety and this was a step you were not quite ready to make but you felt you had to do it anyways - I suspect your professor would be impressed by your resilience and determination.
There is no time to explain so my advice is to go back through posts and look at anything explaining "want to" verses "have to". As far as I know it is the only answer to Agoraphobia in the little time you have. It isn't a replacement for the program but a quick fix to go with it.
I know that with exposure I am supposed to start small, where I am likely to have success. But I do have to do something with a time deadline on it, which for me is big, or I can't graduate. I need to request an appointment with my advisor to transfer credits from a previous degree. I will need to meet with her one on one. I have very bad social anxiety, and reqesting something from an important and busy person is very hard for me, and gives me panic attacks. I have literally been putting this off for years, and now I only have a few days. Advice?
