54 y/o male who was once a competent upbeat educated go-getter. I've been off work (CPA) for 2+ years (but am going back to work in 10 days). I've forced my self to go back to work b/c I feel deep guilt for not accomplishing anything. No motivation, extreme fatigue and dark foreboding thoughts about a hopeless future have clouded me. Am on SSRI, and just spent 6 weeks in the local hospital mental heath ward after a severe depressive session in late February.

 
I've started the 16 week program here (just printed my first Activity Schedule). I like the systematic approach here, and I am desperate to get out of this funk. I am strong willed, and have made up my mind that living this way is intolerable. I have made up my mind to get back to be the kind of person I was earlier in my life: effervescent, upbeat, leader. Although consistent with symptoms described here, harbour serious doubts about my ability to perform competently in the future (like I've "lost it" for good). I have "been up" before, and I want it back. I'm missing lots of life and am wasting my one time trip on this earth.
